We are able to offer free, 24/7 on-site assessment and referral services to our patients. The confidential assessment, conducted by a master’s-level professional or psychiatric nurse, helps us dive into  past behaviors and current symptoms so we can determine the best treatment options for each individual.

Chemical dependency ultimately affects one’s safety, relationships, independence, and overall well-being.  Creekside’s staff has years of experience addressing chemical dependency issues and compassionately guiding individuals toward freedom.

  • Failure to meet important obligations
  • Missing school, work, or other important aspects of one’s life
