Program of Assertive Community Treatment
Research shows that many individuals struggle to continue care after leaving inpatient or partial hospitalization programs due to barriers such as transportation, financial concerns, or anxiety. PACT, or Program of Assertive Community Treatment, helps bridge this gap, significantly reducing hospital readmissions, incarcerations, and medication non-compliance.
PACT is a “hospital without walls,” bringing personalized mental health care directly to the individual —whether at home or within the community. Our goal is to offer flexible, person-centered care for individuals at any stage of their mental health journey.
Our PACT team works with individuals diagnosed with bipolar disorder, schizophrenia, schizoaffective disorders, chronic substance use, and other conditions. We believe that every person has the potential to lead a meaningful life, and we support them in achieving their goals. Our approach ensures that patients are met where they are—both physically and mentally.

Who Benefits from PACT?
PACT is best suited for adults (18+) who:
- Have bipolar disorder, schizophrenia, or other related diagnoses — with or without coexisting conditions like substance use disorders
- Are at risk for repeated hospitalization or in need of recovery supports
- Seek person-centered treatment that is flexible and adaptable to one’s goals and needs
- Struggle with staying on track with their treatment or need help with skills for day-to-day living, including vocational and social skills
- Need individualized care and access to a compassionate mobile treatment team
- Want help taking steps to achieve their goals
Program Details
- Length: A few months to longer-term care, based on the individual’s health and needs
- Requirements: Participants must live in Sullivan County and must complete an intake assessment to be accepted into the program. They also need to be willing to engage in treatment, and allow PACT clinicians into their homes
- Getting Started: Our Assessment & Referral (A&R) team provides no-cost intake assessments 24/7, 365 days a year. Walk-in or schedule via telehealth. After the intake is completed, your information will be sent to the PACT team for review, and a team member will contact you upon acceptance.
